      <image:caption>Homeowner participants are referred to our Age Well at Home programming from partner agencies like NorthPoint Health and Wellness Center in North Minneapolis or Urban Partnership on the east side of St. Paul, as well as participant family and friends. Referrals are sent to our Director of Family Relations, Lauren Kvasnicka. Meeting with our team In a meeting, Lauren walks the participants through the application process, including discussing health and safety concerns in their house, and including issues raised by family members and referral partners. Listening is a vital part of our program’s success. Many of our homeowner participants have experienced trauma and we are sensitive to where they are coming from and help them feel that we hear their concerns and will not do anything without their direction. Building relationships and trust is critical with individuals. We are attentive as they share with us their needs. They know their homes and how they want to navigate their lives.</image:caption>

      <image:title>Stories - Safety Modifications Support Recovery - Maylee S. is proud of the house she has called home for over 30 years. It’s a safe and quiet place to raise her children, befriend her neighbors, and tend to multiple gardens of beautiful flowers.</image:title>
      <image:caption>At age 63, after working many years in a local clinic and nearing retirement, Maylee suffered a stroke that changed her life. She lost the working use of her entire right side and her ability to speak. Not only a traumatic event, but an ill-timed one, as it occurred at the start of the COVID-19 pandemic when many services were shut down. Following her hospital and rehab time, Maylee was confined to her home to recover. She was unable to leave her home on her own, as she could not use her walker on the steep front stairs and walkway.</image:caption>

      <image:title>Stories - Safety Modifications Support Recovery - In September 2020, the Age Well at Home team was able to build a set of wooden steps that are “accessible” for Maylee to use with her walker because they are wider and more shallow than regular stairs. The team collaborated with a local contractor to build a beautiful set of sturdy iron railings alongside the new staircase, providing Maylee with a safe and dependable means to lean against the railings on both sides of the stairs, while gripping her walker securely with her left hand.</image:title>
      <image:caption>Her son, or another family member or friend, could accompany her down the steps if needed, but she gained immediate independence with the installation of the stairs and railing. A comfort height toilet was also installed. This kind of toilet has a stool that is higher than standard height. It enables easier and safer sitting and standing in the bathroom. Lastly, a shower bench was purchased and positioned to enable Maylee to safely sit and shower with a handheld shower wand in her left hand.</image:caption>

      <image:title>Stories - A Community Organizer’s Forever Home - An Age Well at Home on-site visit determined that: Linda’s oven, microwave and refrigerator were breaking down, becoming unsafe to use and expensive due to being highly energy inefficient. She lacked adequate storage space for her pots, pans, and cooking utensils and found it difficult to stoop down on the floor to access them. The front and back storm doors were in extreme disrepair and leaked cold air into the home. There was no railing on the back door stairway entrance leading to the backyard garden and gathering space.</image:title>

Her efforts to purchase her home of 19 years from a neglectful landlord were boosted by a groundswell of support from her neighbors. They gathered over 500 petition signatures to demand that her lease termination process be halted with transfer of ownership to her.</image:title>
      <image:caption>Known as “Miss Linda” to her neighbors, she is everyone’s grandma. Linda is so beloved by her community that they organized meetings with elected officials, contacted media, and held fundraisers to partner with Linda to successfully purchase her “forever home.”</image:caption>

      <image:caption>Marlene also described how painful and difficult it was to get up and out of the chairs in her living room, a good candidate for a motorized lift chair. She described several falls while entering her sunken bedroom from the living room, indicating need for a safety grab bar. Her shower was missing a handheld wand to enable her to shower thoroughly and safely, and she also lacked a railing to the basement to safely do her laundry.</image:caption>

For over 45 years, she provided direct care to seniors and hospice care to people of all ages. Due to her difficulty with walking up and down the stairs to the basement to do laundry and work on her floral arrangements, our team installed a second railing and put in a comfort height toilet so that she didn’t have to try to get back upstairs while working. We replaced her broken washer and dryer with a new set, fixed a light fixture in her basement storage closet, and repaired a non-functioning humidifier.</image:title>

She can no longer play her beloved piano due to the impacts of recent surgeries, but she continues to apply her artistic skills to the creation of beautiful silk flower arrangements that she formerly sold at arts and crafts shows.</image:title>

      <image:caption>Our Age Well at Home team conducted an intake interview with LaJune to assess her needs. She has invested a significant amount of time and resources in her home and wishes to remain there as long as she can to care for her grandchildren. Significant health concerns, however, present a challenge for her. She has poor balance due to weakness in her legs, significant back and knee pain, and she receives treatment for asthma and high blood pressure. LaJune shared that she wrestles with anxiety, and that COVID presented additional barriers in her ability to access the services she needs to maintain her home and her health. An on-site visit identified a list of possible modifications, which LaJune ranked in order of importance to her.
